Youth Urgent Care Appointment Request

Our Youth Urgent Care hours are 11am-7pm daily.

In case of emergency, go to the nearest emergency room or call 911. Please do not proceed with scheduling at this time.

If your child's presentation worsens to an emergency before the time of their appointment, please go to the nearest emergency room or call 911.

What is the main reason for the appointment?

Is your child currently expressing any thoughts of self harm, suicide, or harming others?

Is your child currently at immediate risk of harming themselves or others? (examples include: immediate medical needs, suicide plan or attempt, or suspected overdose)

Is your child experiencing panic attacks, excessive worrying or racing thoughts?

Is your child currently expressing any thoughts of self harm, suicide, or harming others?

Is your child currently at immediate risk of harming themselves or others? (examples include: immediate medical needs, suicide plan or attempt, or suspected overdose)

Is your child currently expressing current aggressive behaviors towards themselves or others?

Is your child currently at immediate risk of harming themselves or others? (examples include: immediate medical needs, suicide plan or attempt, suspected overdose, physical aggression, punching, kicking, destroying property)

Has your child verbalized a plan and/or intent?

Has your child used any substances (alcohol, drugs) recently (last 24 hours)?

Do they require medical attention? (examples include: inability to stay awake, nausea, vomiting, inability to eat or drink, fever)

Do you believe the substance use is related to a behavioral health issue?

Is your child bingeing, purging, restricting eating or excessive eating? (examples include: deliberately not eating meals or snacks, forced vomiting, laxative use)

Has your child fainted, felt dizzy or had a low heart rate recently?

Has your child ingested any harmful substances or taken an overdose?

Is it a medical emergency?

Do you feel you can keep your child safe until their appointment?
